3) Assume all risks and hazards incidental to such participation, including transportation to and from all activities; 4) Understand that is an all volunteer association run by parents of its players, and that I may be asked to participate in volunteer activities in support of the GSAF. ie: help in concession stand, prepare fields, trash pickup, registration, etc.;

5) Waive, release, absolve and indemnify and agree to hold harmless the GIRL’S SOFTBALL ASSOCIATION OF FRANKLIN, it’s Officers, it’s Board of Directors, It’s Sponsors and Contributors, the Coaches and Managers, the Parents, the Players, The Officials, Williamson County Parks and Recreation, and persons ransporting your child(player) to and from any league activities, for any claim arising out of injury to child or parent/guardian;

6) Grant permission to any league official to authorize and obtain medical care and treatment from any licensed physician, hospital, or medical clinic, including major surgery, deemed necessary by a duly licensed physician, should the child become ill or injured while articipating in any and all league activities at any time when neither parent or guardian is available to grant authorization for treatment;
